How to Apply for Membership:
Applicant TO DO checklist

- Print out the application and complete pages 1-4 (can be typed or written NEATLY).
- Complete the essay on a separate sheet. Maximum of 250 words, typed.
Pages 5 and 6 are teacher recommendation forms. Give to TWO of your teachers (current or former). Your teachers will place the recommendation form in Mrs. Valleau's mailbox.
- Page 7 is the Administrator’s form. Give to one of the assistant principals to fill out. They will place in Mrs. Valleau's mailbox.
- Page 8 is an outside recommendation form for leadership or service. Give to someone who is not one of your teachers, along with a blank envelope.
- Retrieve envelope from your outside recommender then place it and everything else in an 8 1⁄2 by 11 manila envelope. On the front of the envelope, you should put your name, my name (Mrs. Valleau) and NHS application.
- Return to Mrs. Valleau’s (Room 100) by the October 22, 2021 deadline.
